Quantifying the Cardiovascular Physiology of REBOA and Partial REBOA: How REBOA Facilitates Resuscitation but Also Strains the Left Ventricle

چکیده
Resuscitative endovascular balloon occlusion of the aorta (REBOA) is increasingly used as an approach to aortic occlusion. This can provide complete or partial and thereby known augment afterload relative extent Despite increasing use this tool, little cardiovascular physiology has been quantified across clinically relevant REBOA transitions: placement occlusive REBOA, then transition partially lastly, removal. The goal study pressure-volume loop analysis direct left coronary artery blood flow measurements capture these transitions in a swine model hemorrhagic shock elucidate underlying physiologic mechanism clinical use. REBOA and catheter-based technology in trauma.
Thank you for your Current Opinion article in last month’s edition of the Journal of Trauma and Acute Care Surgery. The renewed interest in catheter-based hemorrhage control for trauma leads to many more questions than answers. The data for the use of resuscitative endovascular balloon occlusion of the aorta (REBOA) are indeed minimal, and evidence to suggest this procedure to be beneficial for...
متن کامل“REBOA” – Is it Really Safe? A Case with Massive Intracranial Hemorrhage Possibly due to Endovascular Balloon Occlusion of the Aorta (REBOA)
BACKGROUND Non-compressible torso hemorrhage continues to be the leading cause of preventable death in trauma patients. Recent case series report that resuscitative endovascular balloon occlusion of the aorta (REBOA) in the trauma population is a technically feasible method to manage the patients with exsanguinating hemorrhage. On the other hand, it seems that REBOA is being widely promoted pre...
متن کاملPhysiologic tolerance of descending thoracic aortic balloon occlusion in a swine model of hemorrhagic shock.
BACKGROUND Resuscitative endovascular balloon occlusion of the aorta (REBOA) is an emerging technique in trauma; however, the physiologic sequelae have not been well quantified. The objectives of this study were to characterize the burden of reperfusion and organ dysfunction of REBOA incurred during 30 or 90 min of class IV shock in a survivable porcine model of hemorrhage. METHODS After indu...
متن کاملThe role of REBOA in the control of exsanguinating torso hemorrhage.
The management of patients with exsanguinating torso hemorrhage is challenging. Emergency surgery, with the occasional use of resuscitative thoracotomy for patient in extremis, is the current standard. Recent reports of REBOA (resuscitative endovascular balloon occlusion of the aorta) have led to discussions about changing paradigms in the management of patients in both civilian and military ar...
متن کاملResuscitative endovascular balloon occlusion of the aorta (REBOA): an updated review.
In a current scenario where trauma injury and its consequences account for 9% of the worlds causes of death, the management of non-compressible torso hemorrhage can be problematic. With the improvement of medicine, the approach of these patients must be accurate and immediate so that the consequences may be minimal. Therefore, aiming the ideal method, studies have led to the development of Resu...
